Sublingual Gland: Anatomy, Blood Supply, Nerve Supply & Clinical Anatomy

 Introduction

The sublingual gland is the smallest of the three major paired salivary glands. It is located beneath the mucosa of the floor of the mouth, on either side of the tongue.

It produces predominantly mucous saliva, which helps lubricate the oral cavity and facilitates swallowing and speech. Despite its small size, the sublingual gland has important anatomical relationships with the mandible, mylohyoid muscle, submandibular duct, and lingual nerve.

Location of the Sublingual Gland

The sublingual gland is located in the floor of the mouth, beneath the mucosa and on either side of the tongue. It lies in the sublingual fossa on the medial surface of the mandible.

The gland is positioned above the mylohyoid muscle and contributes to the formation of the sublingual fold in the floor of the mouth.

Relations of the Sublingual Gland

The sublingual gland has important relationships with several structures in the floor of the mouth:

* Laterally: Body of the mandible

* Medially: Genioglossus muscle

* Inferiorly: Mylohyoid muscle

* Medially and posteriorly: Submandibular duct and lingual nerve

These relationships are clinically important during surgical procedures involving the floor of the mouth.

Ducts of the Sublingual Gland

The sublingual gland has multiple small ducts, known as the ducts of Rivinus. These ducts open directly into the oral cavity along the sublingual fold.

A larger duct, called the major sublingual duct (Bartholin duct), may also be present. It may open separately into the floor of the mouth or join the submandibular duct.

Blood Supply of the Sublingual Gland

The sublingual gland receives its arterial blood supply mainly from the sublingual artery, a branch of the lingual artery.

It also receives contributions from the submental artery, which is a branch of the facial artery.

Venous blood drains through veins that ultimately join the lingual venous system.

Nerve Supply of the Sublingual Gland

The parasympathetic secretomotor supply to the sublingual gland originates from the facial nerve (cranial nerve VII).

The pathway is:

Facial nerve → Chorda tympani → Lingual nerve → Submandibular ganglion → Sublingual gland

Parasympathetic stimulation increases saliva secretion.

The sympathetic fibers reach the gland through the periarterial plexuses and mainly influence the glandular blood vessels.

Functions of the Sublingual Gland

The main function of the sublingual gland is to produce saliva, which is predominantly mucous in nature.

Its secretion helps to:

* Lubricate the oral cavity

* Facilitate swallowing

* Keep the oral mucosa moist

* Assist speech

* Contribute to oral protection

Clinical Anatomy

Ranula

A ranula is a mucus-filled cystic swelling that usually develops in the floor of the mouth due to leakage or obstruction involving the sublingual gland or its ducts.

It commonly presents as a painless, soft, translucent or bluish swelling beneath the tongue.

A ranula that extends into the neck through or around the mylohyoid muscle is called a plunging ranula.

Sublingual Gland Tumors

Tumors of the sublingual gland are uncommon. A mass in this region requires careful evaluation because the gland is closely related to important structures of the floor of the mouth, including nerves and ducts.
